Unicare Health develops new resource for community care with WA's largest health and aged care provider - Silver Chain

Unicare Health would like to thank Silver Chain for working together with us to develop UniPack – the all new Assistive Technology (AT) equipment pack to support occupational therapists working in the community.

Facilitated by Jasmine Yii, OT at Silver Chain, UniPack has been designed based on meaningful insights provided by the many OTs working at Silver Chain, and the understanding of a client’s common needs and expectations.

UniPack consists of 36 pieces of commonly prescribed AT (devices and equipment) in a handy travel bag, curated to provide Silver Chain’s OTs with the best tools to promote safety, independent living and mobility management within the home.

Silver Chain

With a proud history of serving Australians for over 110 years in Western Australia (WA), Silver Chain is currently WA’s largest health and aged care provider of community care.

Silver Chain plays a significant role in WA’s community care space, specializing in palliative care, wound care, home hospital, chronic and complex services and in-home aged care.

Silver Chain’s team of allied health professionals play a very important role in enabling clients to reach their rehabilitation and reablement goals. Unicare Health supports this role by providing Silver Chain’s therapists and clients with AT (devices and equipment) necessary in supporting a range of interventions and treatments to improve a client’s functional skills.

The Challenge

As Silver Chain’s preferred equipment partner of many years, Unicare Health works together with Silver Chain’s allied health professionals and care co-ordinators to provide assistive technology (AT) equipment and services in the community.

AT (devices and equipment) are necessary in supporting a range of interventions and treatments to improve a client’s functional skills, so they can regain their independence and quality of life.

It is commonly difficult for a client who is experiencing some functional decline to understand how various forms of AT can assist them in performing their daily activities.

Based on a client’s functional assessment, Silver Chain’s OTs would typically call into Unicare to borrow a range of commonly prescribed AT to trial and demonstrate at a client’s home. This is then returned to Unicare after the visit and the required AT ordered for the client through the Unicare-Silver Chain Portal.

Silver Chain’s client base continues to grow and making frequent trips to borrow and return AT for trials and demos with clients has not been an OT’s most productive use of time.

A solution to improve productivity and reduce the amount of time spent dropping into Unicare to borrow and return AT would allow Silver Chain’s OTs to focus on the amazing care they provide for their clients.

The Solution

Facilitated by Jasmine Yii, OT at Silver Chain, Unicare Health and Silver Chain started to work together to develop a new resource we now call UniPack.

UniPack has been designed based on meaningful insights provided by the many OTs working at Silver Chain, and the understanding of a client’s common needs and expectations.

There are 36 pieces of commonly prescribed AT (devices and equipment) included in UniPack, curated to provide Silver Chain’s OTs with the best tools to promote safety, independent living and mobility management within the home.

UniPack stays in every OT’s car, reducing the need for multiple trips to and from Unicare Health to borrow and return equipment.

While UniPack does not contain all the AT necessary to support every aspect of a community client’s needs, it gives both the OT and client a good idea of what could be achieved with AT, and how different forms of AT can work together to increase functional independence around the home and in the community.

The OT can then log into the Unicare-Silverchain Portal and order the required AT of the right size and features for the client.

This solution has reduced the number of trips to and from Unicare and helped Silver Chain and their clients with time and cost savings.
